A blood transfusion can be a very beneficial procedure. It is used in instances where patients lose a large amount of blood due to surgery or serious injury as well as treatment for severe anemia, clotting disorders, hemophilia, and sickle-cell anemia. While this procedure helps thousands it can also be a very dangerous procedure. Mistakes made with blood transfusions can cause severe injury, infection, or even death.

Mistakes such as using the wrong blood type for the patient, the patient is given contaminated blood causing them to develop serious infection and disease, or surgeons wait too long or do not perform a blood transfusion at all causing serious injury or even death to the patient.

Receiving the wrong blood type during a transfusion can cause a condition known as Acute Hemolytic Transfusion Reaction (AHTRs). This condition is the most common cause of death due to blood transfusions and is completely preventable if the health care provider takes necessary precautions to ensure the correct blood type is being distributed to the patient. Contaminated blood that was not properly tested for AIDS, HIV, hepatitis, or other diseases can also end in fatal results.

The fault for a blood transfusion error can be placed on the health care provider performing the procedure and/or the health care facility. Reasons for error to occur with blood transfusions include:
  • Multiple samples being cross-matched
  • Insufficient training for staff members
  • Not storing the blood properly
  • A break in communication between health care providers,
  • Labeling and patient ID mix ups.
